Package includes: New pt. Exam, x-rays, and one customized bleaching upper and lower tray and starter kit of bleaching
gel. Continue to remain an active patient of Aloha Family Dental and you will receive free bleach for life to use in your
trays to keep your smile at it's best. Price does not include an adult cleaning which must be paid separately by patient
at time of visit. **THERE IS ONE CONDITION -You must remain an active patient,
meaning at least one cleaning every six months at which time you will receive one free syringe of bleaching gel to use. Failure
